Who is eligible:
YOU (or any of your friends, family or contacts) who are getting the FIRST vaccine dose at a NYC run vaccine center (scheduled or walk-in) or NYC-at-home appointment.
How to earn $100 for FDC:
While scheduling, or during, your vaccine appointment – be sure to choose FDC (The Flatbush Development Corporation) from the list of referral beneficiaries.

2. To utilize the vaccine referral benefit, patients must sign up and fill out an appointment slot on nyc.gov/vax4nyc/. Note: Walk-up patients can utilize the benefit and support organizations. On-site, patients will be instructed to fill out the form at nyc.gov/vax4nyc. When patients walk up to the vaccination site, they should specifically ask their clinician to use the sponsorship field.
